      This is a request to the FAA for a formal investigation into the failed
      Starship launch.

      Clearly the flight termination system failed.
      
      The FTS only blew holes in the fuel tanks, but the rotation of the
      Starship did not alow the fuel to flow to the open flame of the engines.
      
      Watch closely,  the actual failure of the Starship was due to a
      Raptor engine explosion. 
      
      If the raptor engines had all flamed out, the full Starship could have
      crashed intact.
      
      Please do your homework. 
      
      As evident in the launch pad destruction, Space-x was negligent in their
      incompetent attempt to launch Starship.
      
      The FAA is complicent by not performing a more through investigation into
      the Starship itself, especially the FTS.
      
      Sadly, NASA's critical path to the Mooh has been seriously extended by
      Space-X's failures.
      
      NASA has a strong reputation of producing reliable spacecraft like the
      Artemis.

      I was 5th management layer from the top at IBM, and I fully understand the
      requirements to produce a product that is SAFE, not hazardous.
      
      Tax dollars have been wasted by Space-X's so-called "successful" failures.
      
      One of my first memories is the Apollo 1 failure. May they rest in peace.
